Access to Your Health Records

Updated 8/3/2026

Texas law (Health and Safety Code § 181.105) requires us to give you clear instructions on how to request your health records, contact our licensing authority, and file a consumer complaint. This page provides those instructions.

How to request your health records

You have the right to obtain copies of the health records we maintain for you. To request them:

1. Submit a written request. Send a signed, dated letter to our office at 2806 Mitchell St. Greenville, TX 75402, or email info@drugtestingoftexas.com
with your request. A written request is preferred so we can verify your identity and process it correctly.

2. Include identifying information. Your full name, date of birth, phone number, and a mailing or email address where we can send the records.

3. Specify what you want. Describe the records you need — for example, “all records from January 2024 to present” or “office notes from my visit on [date].”

4. State how you want to receive them. Paper copies mailed to you, electronic copies emailed to you, or pickup at the office.

5. Be aware of fees. Texas law allows us to charge a reasonable cost-based fee for copying and mailing records. We’ll tell you the amount before we produce them.

6. Response time. We’ll respond to your request within 15 business days of receiving it.

If you have questions about your request, call us at 903-481-7250.
